In order to ensure proper enrollment, we require documentation for each student's file and ask that you provide us with the following things as soon as possible:

• Student Application for Admission 
• Application-at-a-Glance
• Birth Certificate and Social Security Number
• Most Recent Report Card
• Certification of Vision, Hearing, and Dental Form
• Two Letters of Reference
• Signed Student Handbook
• Records Request
• Financial Worksheet and Policies
• After-School Program Application
• Uniform Purchased Form
• Obtain from doctor's office or County Health Department:
  • Form 3231 (Certificate of Immunization - Georgia Dept. of Human Resources)
  • Form 3300 (Certificate of Ear, Eye, and Dental Examination)

Please note that all new students are interviewed and tested by a member of the Admissions Committee to ensure present performance, school program adequacy, curriculum, and instructional goals. We will be contacting you to set up the schedule for this experience.
